Introduction:

Peace of Mind Care Management (POMCM) provides advocacy, care coordination, family/caregiver training, education, and support for older adults and adults with disabilities. The purpose of the texting privacy policy is to let our clients know what information we collect, why we collect information, how we store information, and what information we share.

Types of Information Collected:

POMCM collects client and responsible party's names, addresses, email addresses, phone numbers, medical and psychosocial history, and insurance policies information.

Method of Information Collection:

Our client's information is collected from forms that the client and/or responsible party fill out during the client intake process. POMCM will never request personal information via SMS.

Purpose of Data Collection:

Our client's information is collected in order to provide high quality, comprehensive, care management services.

Data Security Measures: 

Our client's information is stored in a secure, HIPAA-compliant, electronic medical record system.

Data Sharing: 

POMCM only shares client information with those who the client, or responsible party, has requested that the information be shared with. POMCM never shares information with third parties, unless required to do so with a valid court order, or if the client has been determined to be an imminent danger to themselves or others. POMCM will never share personal information via SMS.

User Rights: 

Clients are grated access to POMCM's electronic medical record system. If a client or responsible party requests information to be updated or deleted, POMCM will do so immediately.

1- SMS Consent Communication:

Information (Phone Numbers) obtained as part of the SMS consent process will not be shared with third parties for marketing purposes.

 

2- Types of SMS Communications:

If consent has been given to receive text messages from Peace of Mind Care Management, messages may be received related to the following: confirming appointments, emotional support, health education, brief updates, follow-up messages

 

For example: "Hello, this is a reminder of your upcoming appointment with Lisa Carle at your home this Wednesday at 1 pm. Reply STOP to opt out of SMS messaging at any time."

 

3- Message Frequency:

Message frequency may vary depending on the type of communication. For example, up to 10 SMS messages per week may be received related to brief updates

 

4- Potential Fees for SMS Messaging:

Standard message and data rates may apply, depending on the carrier's pricing plan. These fees may vary if the message is sent domestically or internationally.

 

5- Opt-In Method:

Opt-in to receive SMS messages from Peace of Mind Care Management can be done in the following ways: email consent

 

6- Opt-Out Method:

Opting out of receiving SMS messages can be done at any time by replying "STOP" to any SMS message received. Alternatively, direct contact can be made to request removal from the messaging list.

 

7- Help:

Additional Options:

If SMS messages are not desired, the SMS consent box on forms can be left unchecked.
